Low Vision Intake
Office Policies:
If your insurance requires a prior authorization by your physician, it is your responsibility to have that at the time of you visit. Please provide the following form to your physician to request consultation:
Ho Vision Group participates with many major medical insurance plans including: Blue Cross Blue Shield, Medicare, TriWest, and Humana.
We are not in network with any vision plans.
We also work with Vocational Rehabilitation Services and Worker’s Compensation on a case by case basis.
Specialty glasses, low vision aids, and vision rehabilitation therapy are not covered by vision or major medical plans. All costs of any prescribed treatment plan will be provided.
